Help Needed For New Patient Form Research

I am trying to do a research project this semester in my Medical Sociolgoy that analyzes new-patient information forms in hospitals and doctors offices. The idea is to take a look at what information we do ask, what information we don’t ask, and how this process might be socially influenced.
The analysis will simply require that I look at as many forms as I can get my hands on to see what’s being asked and what’s not being asked from a sociological perspective. The names of doctor’s offices or hospitals never enters into the equation (it is effectively “anonymous”). Patient privacy should not be an issue, as the analysis is intended for blank forms only.
What I need help with is actually getting some of these blank, new-patient information forms. I have a few things standing in my way, including a tight work/class schedule I’m already behind on, trouble knowing who to contact in the right places, and a mild phobia of speaking with strangers on telephones, which are the most efficient way to request help.
If any of you know anyone who could get a blank form, provide a blank form, or for some reason has a blank form and you could ask on my behalf, I’d be eternally grateful. It’s getting late in the semester, and the logistical trouble just getting my hands on enough forms to analyze is really holding me back.
The forms can be mailed (I’ll even send a SASE if needed), emailed (if a digital copy exists), or faxed (I can provide my department’s fax number). Thanks to anyone who can help.
